Lifestyle Assessment

The lifestyle assessment is a guide to assist you in determining where you are in your relationship with your surroundings. What are your own behaviors that are calming, stress-free and empowering? This 15-minute assessment is a questionnaire and is not designed to serve as a psychological or mental health tool. By completing it, you may find areas in your environment and behavior that contribute to different outcomes for your well-being. The goal of the assessment is to help you determine what needs your attention or mindfulness.
